The Hub is a collaborative and innovative systems change that aims to bridge the gaps that exist between healthcare and social services by addressing social determinants of health through Community Care Coordination. It is designed to bridge the gap between health care delivery and the social service sector for cost savings, improved population health outcomes, and increased client experience and engagement. This unique collaboration is impressively improving the systems connecting human services with at-risk individuals and families in our community. More information can be found here. Great Rivers HUB was developed and launched on August 1st 2017. Its mission is to facilitate cost savings across health systems and the community, improve population health outcomes, and improve client experience and engagement. This is accomplished through the implementation of the evidence-based Pathways Community HUB model which aims to reduce risk through focus on outcomes. The HUB model is a system change initiative that addresses the ingrained and often siloed system issues that many at-risk populations face today. This is an initiative spreading quickly throughout the nation and this community is leading the movement in the state of Wisconsin. Great Rivers HUB shifts our community’s focus from services provided to outcomes achieved, from provider-led to client-led, and from siloed organizations to community collaboration.
